Travelling for Appointments

To book travel for an appointment on the mainland please:

- download & complete the form below

- scan or take a photo of it

- write your email address & phone number at the top of your appointment letter and scan or take a photo of that

- email both documents to ptc.islay@nhs.scot

Some expenses incurred for travel booked independently can be recovered by completing the form below and emailing it to ptc.islay@nhs.scot.
